Gingival retraction is a critical step before scanning, and the techniques vary significantly based on the patient’s gingival biotype, sulcus depth, and specific needs. Effective Techniques for Thick Biotype Gingival Retraction
Timing of Cord Removal
For patients with a very thick biotype and deep sulcus, the retraction cord can be removed just before scanning. This allows for approximately 30 seconds during which the gingiva remains retracted, providing a clear view of the margins for accurate capture. This brief window is critical for obtaining precise digital impressions without the interference of gingival tissue.
Use of Teflon Tape
For thick biotype, Teflon (PTFE) can be particularly effective. Teflon provides the necessary pressure to retract the gingiva without causing trauma. It is beneficial around implants, inlays, overlays, and other preparations due to its gentle yet effective tissue management properties.
Practical Considerations
Hemostasis
Teflon, or PTFE, is a hydrophobic material that can be beneficial for gingival retraction in thick biotype cases with a healthy, deep sulcus. Teflon does not absorb hemostatic solutions, making it ideal for situations where hemostasis is not a primary concern. In cases with significant bleeding, the contrast benefits of Teflon are diminished as it becomes discolored.
Discolored Margins
The contrast provided by the white Teflon can be advantageous for deep, discolored margins. However, if the margins are in enamel and the enamel is white, the contrast may be less effective. Ensuring the Teflon remains clean and visible is crucial for effective use.
Achieving ideal gingival retraction in patients with a thick biotype is essential for accurate digital impressions and successful restorative outcomes. Techniques such as using Teflon tape and strategically timing the removal of the retraction cord specifically address the needs of thick biotype gingiva.
